Our office is located in the Virginia Dare Center on the NW Corner of Haven Ave and Foothill Ave. We are located in a cluster of four two-story buildings behind the dental/orthodontist office (8004 Haven) and Mountainview Church. We are not visible from the street. Our address is 8002 Haven Ave, Rancho Cucamonga, CA, 91730.

Personal Insurance:
We are able to take some PPO insurances with out of network benefits, but we are out of network with most insurances. We often take Aetna, Blue Cross, United Health Care, Meritain, Loma Linda Risk Management, Healthcare services/ Harrington insurances in our office.
We are in-network with Blue Shield, but are unable to take plans that are processed through a third party (ASH/ American Speciality Health)
We do not accept Medicare, Medi-cal, Healthnet, Kaiser, Humana, IEHP, Landmark or any HMO insurances.
To be sure, please send us your Insurance Information so we can verify it for you. Every plan is different.
Personal Injury:
We accept Medical Pay from Car Insurance and select Liens. We can also provide you the superbill for you to claim with the responsible party.
Workers Compensation:
We can usually treat you if you have authorization for acupuncture/chiropractic treatment but we will need to check with your adjuster.Paperwork
Once booking an appointment, do not forget to submit your intake paperwork. It will be sent with you with your appointment confirmation.- If you are coming due to injuries from a car accident, please also fill out the Personal Injury Form.
